In the Contact Applicant section, complete the following fields:

Title	Select the applicant’s title from the drop-down list.
First Name	Enter the applicant’s current legal first name.
Middle Names	Enter any middle names the applicant may have.
Family Name	Enter the applicant’s current legal family name.

If the applicant has changed their name, complete the following details:

Original First Name	Enter the first name on the applicant’s birth certificate.
Original Middle Names	Enter any middle names on the applicant’s birth certificate.
Original Family Name	Enter the family name on the applicant’s birth certificate.
Other Name Used?	Select 'Yes' if the applicant has previously used another name. certificate.

Complete the birth details:

Date of Birth	Enter the date of birth of the applicant, in DD/MM/YYYY format. Note: Entering an applicant under the age of 18 will cause the application to be rejected (for NT jurisdiction) or suspended (for all other jurisdictions).
State/Territory of Birth	Enter the town or suburb where the applicant was born.
Country of Birth	Select the country where the applicant was born.
Contact Phone	Enter the applicant’s daytime contact telephone number. Note: This field will not appear if you are in the Add Applicant page.
Email Adress	Enter the applicant's email address
